- If you got a Notice in the mail, you were identified as a likely Class Member
How to file
- Find the Unique ID and PIN printed on the Notice you got in the mail
- Go to the online claim form and log in with your Unique ID and PIN
- Complete and submit your Claim Form by November 10, 2026
- Save the confirmation email and code you receive after filing
- Or fill out, sign, and date a paper Claim Form and mail it postmarked by November 10, 2026, to Wellstar Pixel Settlement, Settlement Administrator, P.O. Box 2197, Portland, OR 97208-2197

- If you do nothing, you get no money and still give up your right to sue Wellstar over these claims
- Opt-outs must be mailed; you cannot opt out by phone or email, and mass opt-outs are not allowed
- If you opted out by mistake, contact the Settlement Administrator to withdraw it and file a claim
- Class Counsel will ask for up to one-third of the fund in attorneys' fees, plus costs and $2,500 service awards for each Class Representative
- Leftover funds go to Good Samaritan Health Center of Cobb, subject to Court approval
