- You need the LoginID and PIN from your mailed notice to file online. Email the administrator with your full name and mailing address if you cannot find them.
- Cash benefits share a $385,000 cap. If claims add up to more than that, every payment is cut by the same percentage.
- Credit monitoring and identity restoration are not part of that cap.
- The $70 Alternative Cash Payment cannot be combined with ordinary loss, extraordinary loss, or lost time claims.
- Attested Lost Time can be combined with loss claims, but not with the $70 payment.
