$50 No-Proof or Up to $5,000: Community Dental Care Settlement

What happened

Class period: Documented losses must have occurred between December 20, 2024 and October 19, 2026

• Community Dental Care (CD) is a Minnesota nonprofit dental services provider
• In December 2024, an unauthorized actor carried out a targeted cyberattack on CD computer systems
• Files accessed in the attack may have contained names, health insurance information, dates of birth, medical information, and Social Security numbers
• CD denies wrongdoing and denies it broke any laws. The Court has not decided who is right
• Both sides agreed to settle to avoid the cost and risk of continued litigation
• The case is In re Community Dental Care, Case No. 62-CV-25-2490, in the District Court for Ramsey County, Minnesota

How to file

1. Go to https://www.cddatabreach.com/form/claim to file online, which is the fastest option
2. Choose either Cash Payment A (documented losses up to $5,000) or Cash Payment B (flat $50). You cannot claim both
3. Upload receipts, bank statements, or other proof if you claim documented losses
4. To file by mail, download the Claim Form from the official site and mail it to CD Data Breach Settlement, c/o Settlement Administrator, P.O. Box 25226, Santa Ana, CA 92799-9958
5. Mailed claims must be postmarked by October 19, 2026
6. Enroll separately in the two years of medical and credit monitoring using the enrollment code mailed to you by postcard. Call the administrator if you no longer have the code

Proof required

• No proof is needed for the flat $50 Alternative Cash Payment
• Proof is required for the Documented Loss Payment of up to $5,000
• Send bank statements, receipts, or similar records showing the date and amount of each loss
• Self-prepared notes can support other proof but are not enough on their own
• Losses already reimbursed by a third party cannot be claimed
• Lost time and other non-monetary losses do not qualify

Payment timing

Payments are issued after the Court grants final approval at the November 23, 2026 hearing and any appeals are resolved. There is no set payment date.

• Doing nothing gets you no payment and no benefits, and you still give up the right to sue CD over this breach
• Opting out by September 24, 2026 means no benefits but keeps your right to sue on your own
• Objecting is different from opting out. Objecting keeps you in the class while telling the Court what you dislike about the deal
• Class Counsel will ask the Court to approve $400,000 in fees and costs and $2,500 service awards for each Class Representative, all paid by CD
