• You did not previously opt out of this Settlement Class

How to file

1. Go to https://chantixsettlement.com/file-a-claim/
2. Individual consumers click "File a Consumer Claim Online" and complete the form
3. Enter the amount you paid for Chantix and attach any purchase records you have
4. To file on paper instead, download the Consumer Claim Form and mail it to Chantix Settlement, c/o A.B. Data, Ltd., P.O. Box 173137, Milwaukee, WI 53217
5. Health plans and insurers use the separate TPP claim form, TPP filing template, and eligible Chantix NDC list on the same page
6. Online claims are due by 11:59 p.m. EST on September 14, 2026. Mailed claims must be postmarked by September 14, 2026

Proof required

• Your payment is based on the dollar amount you paid, so the claim form asks for that figure
• Include purchase records with your claim if you have them, such as pharmacy receipts, printouts, or insurance statements
• The Settlement Administrator may ask for more purchase documentation after you file
• A claim can be rejected if you do not send requested documents on time, so keep your records until you are paid
• Third-party payors must submit documented payment data using the TPP filing template

Payment timing

Payments go out only if the Court approves the settlement at the October 13, 2026 hearing. After that, appeals can take time, and the Settlement Administrator still has to process every claim. The official notice asks for patience and does not give a payment date or list the payment methods.

• Consumers split up to 20 percent of the fund after fees and costs. The rest goes to third-party payors such as insurers and health plans
• Your individual payment cannot be more than what you actually paid for Chantix
• Payments are pro rata, so the more valid claims filed, the smaller each share
