• Blue Cross Blue Shield Healthcare Plan of Georgia, Inc. and Anthem Insurance Companies, Inc. (together, "Anthem") were sued in federal court in Nashville. The case is Taylor, et al. v. Blue Cross Blue Shield Healthcare Plan of Georgia, Inc. and Anthem Insurance Companies, Inc., Case No. 3:23-cv-00541, before Judge William L. Campbell Jr.
• Proton beam radiation therapy (PBRT) is a type of radiation treatment used for prostate cancer.
• The lawsuit says Anthem denied coverage for PBRT by calling it experimental, investigational, unproven, or not sufficiently proven to work.
• The plaintiffs say those denials broke ERISA Section 502(a)(1)(B), the federal law that governs employer-sponsored health plans.
• Anthem denies the claims and denies any wrongdoing. The court did not rule on who was right.

2. Gather documentation showing what you paid out of pocket for PBRT
3. Complete the form and attach your documents
4. Mail the packet to the address printed on the Claim Form
5. Make sure it is postmarked no later than October 10, 2026

Proof required

• Documentation showing you incurred a Qualifying Payment, meaning money you paid out of pocket for PBRT for prostate cancer
• The documentation must show the provider's name
• The provider's address
• The date of service
• A description of the services provided
• The provider charges you paid out of pocket

Payment timing

Payments go out after the settlement becomes final. The court granted final approval on August 21, 2026, but the notice warns that appeals are possible and can take more than a year to resolve. Your payment amount is based on your total Qualifying Payments. Attorneys' fees of up to 33.33% of the $3,625,000, plus costs and a $10,000 service award for each of the two class representatives, are deducted from the fund first. The notice does not state the payment method.
