- Purchases were made in one of the eligible states or D.C. listed above
- Products were purchased for personal, family, or beneficiary consumption -- not for resale
- Health plans, insurers, and other third-party payors that reimbursed members for QVAR purchases may also qualify under a separate claim process
- You are not Teva, a Teva affiliate, a federal or state government entity, or someone who has already excluded yourself from the class
How to file
- Visit the official settlement website at qvarantitrustsettlement.com and click "File a Claim"
- Individual consumers should complete the Consumer Claim Form; health plans and insurers should complete the Third-Party Payor Claim Form
