Who may qualify
- You paid any amount for a generic metformin-containing drug intended for personal or household use
- The drug was manufactured, distributed, or sold by Teva, Granules, or Heritage
- Your purchase occurred between July 20, 2015 and June 2, 2020
- You are a person or entity in the United States or its territories and possessions
- Third-party payors -- such as health insurance companies and self-insured health and welfare plans -- that paid or reimbursed metformin purchases during that period also qualify
How to file
- Visit the official settlement website at inremetforminsettlement.com/file-a-claim/
- File your claim online by July 10, 2026, or download a paper Claim Form and mail it so it is received no later than July 10, 2026
- Third-party payors should use the separate TPP Claim Form and include transaction data if your claim is $100,000 or more
- You may also request a paper form be mailed to you by calling 1-866-302-6835

Proof required
- Individual consumers are not required to submit documentation at the time of filing, but should retain any receipts, invoices, or pharmacy records from the class period
- Third-party payors filing claims of $100,000 or more must include transaction data; the Claims Administrator may also request it for smaller TPP claims
- The Claims Administrator may request supporting documentation from any claimant after submission, so keep your records on hand
Payment timing
Payments will be distributed on a pro-rata basis after the Court grants final approval and any appeals are resolved.
