How to file
- Go to the official claim site and click "Submit a Claim"
- Log in with the Unique ID and PIN printed on your Postcard Notice, or contact the administrator if you cannot find them
- Complete the Claim Form, then save the confirmation code emailed to you after submission
- Submit online by August 16, 2026, or mail a completed Claim Form postmarked by the same date
