What happened

Class period: Eligibility is defined by notification rather than a date range. You qualify if Deanco notified you on or around November 21, 2023 that your information may have been compromised in the May 1, 2023 Data Security Incident.

• On May 1, 2023, Deanco Healthcare, which operates Mission Community Hospital in Los Angeles, was hit by a cyberattack that breached its IT network.
• A threat actor accessed database files containing patients' addresses, dates of birth, Social Security numbers, driver's license numbers, financial account information, health insurance plan member IDs, claims data, and clinical care information.
• Deanco began notifying affected individuals on or around November 21, 2023.
• The settlement resolves claims that Deanco failed to adequately protect this information. Deanco denies any wrongdoing, and no court has ruled that it broke the law.

How to file

1. Go to the official settlement website at deancodatabreachsettlement.com
2. Click "Submit Claim" and complete the online Claim Form, or download and mail it
3. Enter the Unique ID number from your mailed or emailed notice (call (833) 319-2344 if you lost it)
4. Select which benefit or benefits you want, attach documentation if claiming a Documented Loss Payment, and submit online or postmark by August 12, 2026

Proof required

• For Medical Monitoring, the California Statutory Payment, or the Residual Cash Payment: no documents needed, just your Unique ID (plus a sworn attestation of California residency for the $100 payment)
• For a Documented Loss Payment: reasonable documentation such as credit card statements, bank statements, invoices, telephone records, or receipts for losses on or after May 1, 2023
• Documented Loss claims must be filed online or by mail, not with the tear-off postcard form

Payment timing

Payments are made only after the Court grants final approval (hearing set for September 9, 2026). If approved with no appeals, eligible claimants should begin receiving payments by January 2027, by check, PayPal, Venmo, or another digital option. Medical monitoring enrollment instructions arrive shortly after the Settlement becomes effective, expected October 2026.
