Class period: Documented losses must have occurred between July 19, 2024 and June 29, 2026. The underlying data incident occurred in August 2024.

  • In August 2024, Alera Group, Inc. -- a national insurance brokerage and employee benefits firm -- suffered a cyberattack on its computer systems
  • Certain files containing Private Information were potentially accessed by unauthorized parties during the incident

  3. Complete and submit the online claim form by June 29, 2026
  4. If you prefer to file by mail, download the paper Claim Form from the official site and mail it to the Settlement Administrator with a postmark no later than June 29, 2026

Proof required

  • For the flat \~$50 alternate cash payment: no documentation required
