Proof required
- Alternative Cash Payment ($40 or $100): no documentation required
- Lost Time Payment (up to $100): attestation under penalty of perjury of time spent and your hourly rate; no third-party documents required
- Out-of-Pocket Expenses (up to $500): receipts or third-party records documenting costs; self-prepared documents such as handwritten receipts are not sufficient on their own
- Extraordinary Loss (up to $5,000): documentation verifying identity theft or fraud resulting from the breach; self-prepared documents are not sufficient on their own
Payment timing

- The aggregate cap on cash payments is $1,000,000. If total approved claims exceed this cap, individual payouts will be reduced on a pro rata basis
