- OR you completed a Health Risk Assessment between November 2, 2020 and January 31, 2021, OR between February 1, 2021 and April 30, 2021

- Click "Submit a Claim" and complete the online form -- must be submitted by June 15, 2026
- Alternatively, download the Claim Form from the site and mail it to the Settlement Administrator -- must be postmarked by June 15, 2026
