How to file
- Visit the official settlement website at excelsiordatasettlement.com and click "Start Your Claim"
- Log in using the Unique ID and PIN printed on the Settlement Notice you received by mail (if you lost your notice, call 1-877-327-7791)
- Complete and submit your Claim Form online by June 11, 2026
- Alternatively, fill out, sign, and date a paper Claim Form and mail it to the Settlement Administrator, postmarked by June 11, 2026

- For the Documented Loss Payment: receipts, phone records, email correspondence, or other documentation showing out-of-pocket losses connected to the breach

- You do NOT need to file a claim to receive free Credit Monitoring. Your activation code is printed on your Settlement Notice and can be used after final court approval.
