Who may qualify
- Settlement Subclass 1: Were a Northwell patient AND logged into the FollowMyHealth patient portal between January 1, 2020, and December 31, 2023, OR booked an appointment on northwell.edu during that same period
- Settlement Subclass 2: Were a Northwell patient between January 1, 2020, and July 25, 2024, and do not qualify for Subclass 1
- Are not a Northwell officer, director, legal counsel involved in the case, or a member of the judiciary (again, your honor, sorry)
How to file
- Visit the official claim form at https://www.nwpixelsettlement.com/form/claim/
- Log in with your Notice ID and PIN from the settlement email you received
- Complete and submit the form online by 11:59 p.m. ET on April 20, 2026, OR download and mail a paper Claim Form postmarked no later than April 20, 2026
- If you can't locate your Notice ID and PIN, email info@NWPixelSettlement.com with your full name and mailing address to request them
Proof required
- No documentation of damages is required. You must provide your Notice ID and PIN to verify class membership.
Payment timing
Eligible claims will be paid after the Court grants final approval at the April 21, 2026 hearing and after any appeals are resolved. Payments will be issued by check unless you choose electronic payment. Checks expire 180 days after issuance.
