What happened

Class period: The data breach occurred on September 29, 2023. All U.S. residents whose personal information was potentially compromised in this incident are included in the Settlement Class.

On September 29, 2023, Cardiovascular Consultants Ltd. (CVC) -- a cardiac care provider -- suffered a data breach that exposed the kind of information you really don't want floating around: names, Social Security numbers, addresses, dates of birth, driver's license and state ID numbers, health insurance details, and medical records. CVC began notifying affected individuals on December 2, 2023.

A class action lawsuit followed in Arizona's Maricopa County Superior Court. CVC denies any wrongdoing but agreed to a $3,850,000 settlement to resolve the case. If your personal or health information was potentially compromised, you may be entitled to cash and other benefits -- but only if you file a claim.
