  4. Upload any supporting documentation if claiming documented losses (files must be under 20 MB each; accepted formats include jpg, png, pdf, doc, and others)
  5. Submit online by June 18, 2026 -- or download a paper claim form, sign and date it, attach documentation, and mail it postmarked no later than June 18, 2026
